donation pg 2 7-14-22

Donation
Members of Shannon Labonte’s family recently made another generous donation in her memory.  The funds were some of the proceeds from the recent Shannon Labonte Memorial Artisan and Craft Fair. Shannon was a very special person and frequent library customer who died in 2017.  She was a preschool teacher at Country Kids Child Care in Putnam and had a heart for children and learning. The funds will used to purchase a collection of Wonderbooks, which are a new format of children’s books. Wonderbooks bring audio and print together for literacy and fun, two things that Shannon adored. PUTNAM — Day Kimball Medical Group (DKMG), a division of Day Kimball Healthcare (DKH), was awarded recognition as a Patient-Centered Medical Home™ (PCMH™) from the National Committee for Quality Assurance (NCQA), a private, non-profit organization dedicated to improving health care quality.
“We are truly honored to be recognized by the NCQA for our dedication to providing the highest quality of care to each patient at each interaction,” said Kyle Kramer, chief executive officer, Day Kimball Healthcare.
“A key component to accomplishing this recognition includes our commitment and focus on the patient, particularly around gaining an understanding of, and respect for, their unique needs for care coordination.
"Our team-based approach ensures that DKMG’s primary and specialty care clinicians share information, and use evidence-based medicine and clinical decision-support tools to guide shared decision making with patients and families.”
The Patient-Centered Medical Home (PCMH) Recognition program highlights medical practices that place patients at the forefront of care, where continuous quality improvement is prioritized.
The PCMH program recognizes primary care providers who have demonstrated effectively coordinated care among all of their patients’ health care providers.
The model benefits patients by delivering accessible services with shorter waiting times for urgent needs, enhanced in-person hours, around-the-clock telephone or electronic access to a member of the team, and alternative methods of communication such as email or telephone care.
This improved access helps them to better understand their treatment options and better manage chronic conditions while creating a partnership with their provider.
In order to achieve recognition as a PCMH, DKMG’s outpatient locations met a comprehensive range of performance benchmarks for patient care, including effective care planning, coordination, and access to care, as well as measuring performance and quality.
Patient-centered medical homes provide a care model that builds stronger relationships with patients, improves their experience, and reduces unnecessary emergency room visits and hospitalizations.
They aim to ensure that all patients, regardless of factors like socioeconomic stability, access to transportation, or the community in which they live, receive access to the health care they need.

DAYVILLE — Laurie Plantier, LPN, was honored as the May Employee of the Month award at Westview Health Care Center.  A Licensed Practical Nurse, holds multiple degrees and certifications across various healthcare pursuits.  She started at Westview as a CNA in 1989 and worked at the Dayville skilled nursing facility for about 12 years. She returned to Licensed Practical Nursing which required her to review her knowledge and skills; accomplished by completing some specific educational and clinical course work.  She worked with Westview to fulfill her clinical hours necessary for the license renewal.  After renewing her Practical Nursing License, she resumed caring for patients in the skilled nursing setting of Westview.  At the same time, enhancing her knowledge and skills to learn more about breakthrough treatments by working towards a degree from Three Rivers Community College as a Registered Nurse.  She is especially interested in learning to perform infusion medicine, developing even greater wound care methods, and how to perform more advanced technical treatments.
Away from Westview, Laurie adores her cats and likes to take time to birdwatch; especially loving the sight of raptors.  She is an avid reader, enjoying crime and mystery novels as well as educational journals and science magazines.  As an exercise to keep her mind active, she loves to do puzzles and word searches.
